1. Pick the Super Plan or Income Stream that suits your personal needs.
 
2. Fill in your Membership Application form:
 
  • Personal Plan or Income Streams - application forms are found in the back of the relevant Product Disclosure Statement

  • Super Saver members - Your employer registers you when you start work. You will receive a confirmation letter from us once you've been registered.
 
3. Send your completed form to: PO Box 18041, Collins Street East, Melbourne VIC 8003.
